Sometimes a young person simply needs a welcoming place to connect, build confidence, and discover new ways to support their well-being.
This referral form is intended for school counselors, teachers, therapists, pediatricians, social workers, and other professionals who work with teens and young adults who may benefit from The Noble Path Foundation’s free programs and supportive community.

Please use the form below to tell us about the young person you are referring and why you believe TNPF may be a good fit. Our team will review the referral and follow up to help the young person and their family learn more about our programs and next steps.

This form is for professional referrals only. Parents and caregivers who would like to enroll their own child should visit our Register page. A referral does not complete enrollment; all participants must register through PushPress and complete the required waiver before attending.

Please confirm that the young person and, when applicable, their parent or guardian are aware of the referral and have agreed to be contacted by The Noble Path Foundation.
